Here are the answers from the Scottish Deerhound Club of America:

2-eastern US, Midwest, Western US - rotates every year.  2004 in Vermont
3-yes
4-Obed, Agility, Lure Coursing Field trials,other[LGRA racing, Versatility, various fun contests]
5-people must apply via application and be voted in for full membership.
 
Note - yes please send a copy of the poll results - I am curious also.
